Sig., a-b, A-R, aa-ii, kk⁶, with the catchword "Certaine" on verso of kk⁶ (the first ed. first issue has the quire kk complete the last two leaves, containing the beginning of "Certaine Godly prayers, etc." being usually cancelled, as, by order of the King, these prayers were omitted. Cf. B.M. Liturgies, col. 645.
