Pierpont Morgan from Gruel in 1909; J.P. Morgan (1867-1943).

Ms. written and illuminated in Milan, Italy (calendar for use of Ravenna), in or just before 1386 (table of lunar and solar computations begins with the year 1386).
Artist: a follower of Giovannino de' Grassi.
