Thomas Denman was a man-midwife and one of three who attended to Princess Caroline at the birth of Princess Charlotte.
With a note on the verso "Supposed to be autograph / of George 4th on the night / of the birth of the Princess / Charlotte;" it appears to be the autograph of George IV.
