A note on the last page indicates this Order of Service was used for the consecration of St. Peter's Church, Westchester, December 9, 1795.
According to the "Commentary on the American Prayer Book" by Marion J. Hatchett (New York: HarperCollins Publishers, 1995), p. 542, this form used by Bishop Prevoost, was a "revision of the 1712 Convocation form" and was "probably the first consecration of a church according to this form in this country."
