Giving a detailed account of his illness; describing his symptoms, at length and in detail, for his self-diagnosis of dyspnea and the related issues of deglution; relating details of the doctors he has consulted, their recommendations and the medications he is taking; discussing his house on the Thames and saying it unlikely that it's proximity to the damp is the cause of his illness; concluding "...appetite gradually diminishing though I still eat sufficient, grow thinner & weaker, yet still put on a good face on a very case;" sending his regards to Mrs. Sims and his family.
